MEMBERSHIP CANCELLATION AND FREEZE POLICY

• We hate to see you go but if you must we ask you abide by the cancellation policy outlined in the membership agreement. Here are the specifics:
• You must provide a 15-day written notice. We provide you 15 days from your final payment, so you do not lose out on any time to use the fitness center.
• We ask for you to come to the gym in person so we can provide you a written receipt of cancellation.
• If your maintenance fee is due during the 30-day period, you will still be obligated to pay it.
• You must pay any fee that'll will apply to your membership plan.

• Members may put a hold or “freeze” on the membership for Cost of $5
• If you paid in full for your membership your time is added to the membership when it expires.
• If you are paying monthly, we just stop the payments.
• We do require you to fill out a Membership Freeze Form, so we both have record of it.
• Will I have to pay a start-up fee when I resume ?
  No, there is no start-up or enrollment fee to resume after Stay & Save. Your Premier status will be reactivated based upon a pro-rated reactivation price, which is determined by the level of membership you have and the day you reactivate
• Do I keep my original monthly membership fee when I resume ?
  Yes, your monthly membership rate stays the same after reactivating your account.
• Can I workout while on Hold or Freeze?
  Yes, only $10 for a day pass.

• You can join Mayfield Fitness starting at the age 13 with a parent/legal guardian's permission; they must present with you during the sign-up process.
•  Members aged 13-14 must be accompanied by parent or guardian when they work out.
• Members who are 15-17 years old must have a signed waiver from a parent or guardian on file 

Guest must be 18 years old with a valid ID, unless they're accompanied by a parent or guardian.
